Welcome to our practice!  To help make a smooth transition, we recommend on your first visit, that you please bring the following:

 

• your insurance card

• any medications you are currently taking

• completed New Patient Forms (see below)

• please try to arrive at least 15 minutes prior to your scheduled appointment.

When you arrive, we will create your personal permanent medical record.  To facilitate this process, we ask that you complete a medical history and registration form so that we can get you registered into our system.  You may download these by clicking on the link to the right and bring your completed forms with you to your first appointment.

If you have been under the care of another doctor, we may ask your permission to transfer some of your medical records.
